Primary Home Care (PHC) is a non-technical, medically related personal care service prescribed by a physician as part of a client's plan of care. PHC is available to eligible clients whose health problems cause them to be functionally limited in performing activities of daily living. Patient service is provided by a supervised Personal Care Attendant, who performs tasks based on individual need.
Qualifications are through Medicaid and are based on income, assets, and health conditions that cause functional limitations in performing Activities of Daily Living (ADL’s).
Fee Structure Details:
PAID BY MEDICAID, private pay or private insurance.
